Vulnerable adults in Hounslow are being given access to “floating support.”

The service sees keyworkers build relationships with vulnerable residents to help them live independent lives.

Its aim is to help people avoid hospital admissions, having to rely on social care services, or end up in the criminal justice system.

The service is run by charity Thames Reach and available to residents aged 18 and over who are classified as vulnerable.
