The star of the hit BBC show formed a Dementia Choir after her grandmother was diagnosed with the disease.
An estimated 17,300 people are currently living with dementia in Surrey.
The Surrey Memory Walk will take place at Painshill in Cobham on Sunday October the 1st with others planned for different regions.
