Hounslow council spends over £400 million on health and social care but it says more is needed to help it cope with an ageing population.
The authority says it needs government help to stop costs spiralling past £560 million annually.
The council’s health lead has written to the new Labour Health Secretary, Wes Streeting, about the possibility of working together.
The authority wants to develop a “central frailty hub” to enable residents to be treated within their own communities.
It’s hoped this would ease pressure on hospitals.
On Thursday, Sir Keir Starmer warned the NHS must “reform or die.”
