Ministers are being warned lessons must be learned from a series of self-harm related prison deaths over the last three years.

Hammersmith MP Andy Slaughter says an investigation into 10 self-inflicted deaths in Wormwood Scrubs since 2018 is taking far too long.

It comes as the government’s launched an inquiry into the conditions of Wandsworth Prison following the escape of terror suspect Daniel Khalife.
