Inspectors found problems with managing safety risks, responding to concerns and tackling bullying and discrimination against staff.
They also raised concerns about maternity, surgery and emergency care, as well as the condition of some hospital buildings.
However, the trust was praised for working with other organisations and reducing waiting times and hospital admissions.
