A regular complaint nationwide is a perceived lack of coordination between different healthcare services.
The project will launch on Monday and involve all departments across Kingston and Richmond NHS Foundation Trust.
It will aim to improve the flow of patient information between GPs, hospitals and other healthcare organisations.
