Hounslow Council and NHS partners have launched a pilot helping 400 residents with complex health needs stay well at home.

Hounslow Care Together is a team of GPs, nurses, and support workers providing home visits and care plans to cut hospital visits.

The scheme is funded by the Hounslow Borough-Based Partnership and will be evaluated for a wider rollout.

Residents can access support and guidance through their local neighbourhood care teams.
