The Connected Communities plan will help unpaid carers, families facing financial hardship, isolated individuals, and older people.
It includes creating inclusive community hubs, introducing Area Coordinators, and working closely with voluntary groups.
The programme aims to save £1 million annually to reinvest in frontline services.
