Runnymede Borough Council has decided to sell off one of its community halls for healthcare use.
The site in Thorpe Road will become the new Hythe Medical Practice, with contracts due to be exchanged by the end of the year.
Around 7,600 patients have been based in temporary buildings next to the centre for 15 years.
