The council has approved the scheme for Maswell Park and Heart of Hounslow health centres.
Two ‘Hounslow Care Together’ teams will be created which will each include a GP, paramedic or nurse and a floating support worker.
It’s hoped the teams will help reduce the waiting times and referrals to hospitals.
