Fulham Good Neighbours offers practical support, social activities, meals and exercise classes to help tackle loneliness.
It is temporarily moving its services from Rosaline Hall to Dawes Road while its centre is refurbished.
The charity is one of four chosen by the Mayor of Hammersmith and Fulham, Sharon Holder, to receive her support this year.
