The charity says both the Emergency Care Provision and the End of Life support service have now been withdrawn.
The charity says the “termination of Emergency Care Planning service gives only 3 weeks’ notice, leaving almost 1,500 carers who have existing emergency plans without the safety net they were relying on should a crisis arise.”
The charity claims the decision was made with no consultation.
