Justice Secretary David Lammy has been answering questions in the Commons.

The incidents include the wrongful release of two men from Wandsworth Prison who have since been returned.

The news means 13 inmates a month are being accidentally released from prisons in England and Wales.

Vanessa Frake-Harris is a former governor and also former Head of Security and Operations at Wormwood Scrubs Prison in Shepherds Bush.

She says accidental release is a small part of what’s wrong.
