The report from The Prisons and Probation Ombudsman says staff repeatedly missed opportunities to help 36-year-old Rajwinder Singh.
He took his own life 12 days after arriving at Wandsworth in June 2023.
The report says he had a history of attempted suicide and self-harm and depression, for which he was prescribed medication.
Mr Singh was one of five prisoners who took their own lives at Wandsworth in 2023.
The reports says healthcare staff failed to adequately assess or support his deteriorating mental health.
The Ombudsman, Adrian Usher, concluded he remains “extremely concerned about prisoner care at Wandsworth.”
